Patient Information

Booking an Appointment

Appointments are booked by phone at 403-485-2216 during clinic hours (Monday to Friday, 8:30 am to 4:30 pm). When booking, please let our staff know briefly what the visit is for so the right amount of time can be set aside.

  • If you have several concerns, mention this when booking so a longer appointment can be arranged.

  • Arrive 10 minutes early for your first visit or if your information has changed.

  • Cancellations: please give at least 24 hours' notice so the time can be offered to another patient.

What to Bring

  • Your Alberta Health Care card

  • Photo identification (first visit)

  • A current list of your medications, or the medication bottles themselves

  • Any relevant forms (e.g., driver medical, insurance, WCB paperwork)

Prescription Renewals

The best way to renew a prescription is to book an appointment before your medication runs out. Regular medication reviews are an important part of safe care. Your pharmacy may also fax a renewal request to the clinic at 403-485-2218; please allow a few business days for processing.

Test Results

You will be contacted if your results require follow-up. If you wish to review results, please book a follow-up appointment. Many lab results are also available to you directly through MyHealth Records (MyHealth Alberta).

Medical Records & Privacy

Your health information is protected under Alberta's Health Information Act. Records are released only with your written consent or as permitted by law. To request a transfer of records (for example, if you are moving), please contact the clinic; an administrative fee may apply.

Respectful Environment

Our staff and physicians are committed to treating every patient with courtesy and respect, and we ask the same in return. Abusive language or behaviour toward staff will not be tolerated.

Urgent concerns: If you are seriously ill or injured, do not wait for an appointment — go to the Emergency Department at the Vulcan Community Health Centre or call 911. For 24/7 health advice, call Health Link at 811.
